BH Network Plan (BHN providers) Annual OOP max of $3,000/$6,000 (individual/family) Father, mother, daughter, son on policy. The son meets $3,000 OOP maximum. He does not pay for medical services for the remaining calendar year.

BH Savings Account Plan (BHN providers). Annual OOP max of $5,550/$11,100 (individual/family) Father, mother, daughter, son on policy. The Father met $5,550 OOP maximum. The plan will pay at 100% for the rest of the family for the rest of the year.

The BH Extended Plan (BHN providers) Annual OOP max of $3,000/$6,000 (individual/family) Father, mother, daughter, son on policy. The son met $3,000 OOP maximum. The father met $3,000 OOP maximum. The plan pays at 100% for the rest of the family for the rest of the year.
